FEPTO Membership Application Form
Training Organizations OR Accrediting Organizations
EXPLANATIONS:
1. Your formal application will be requested by the end of September following your introductory presentation in the Annual Meeting of FEPTO. However, we highly recommend that you send it while preparing for your presentation - one year before being voted in General Assembly during the Annual Meeting - (P.S. Look at “The Guidelines for Presentation by Applying Organizations” “The Formal Procedure of Application for Membership”, “Evaluation and Voting Procedure”)
2. You should fill in only one, either the form (I) for the “Training Organization ” OR the form (II) for the “Accrediting Organization”. An accrediting organization is an organization usually accrediting the members of more then one training organizations in the country.
3. If the official address for your Organization is different from your candidate FEPTO representative’s postal address, mention it seperately in the form.
4. In the item 7 of the form I, we ask for the training program of your Training Organization and ask you to compare it quantatively with the Minimal Training Standardts (MTS) of FEPTO.
5. After filling in the form you should send it to the Chair of the Membership Committee of FEPTO along with photocopies of certificates of your main trainers and 2 recommendation letters. Your application will be inquired by Membership Committe members and may further be discussed within the Council of FEPTO. You’ll be contacted by the Chair of the Membership Committee several times through the process.
6. There is an application fee for membership. Please contact to the Treasurer of FEPTO before or during your presentation year in the Annual Meeting.
